Fee-for-service care
Payment at the time of your visit
We are a fee-for-service practice. Payment is due at the time of service and can be made with cash, credit/debit cards, or HSA/FSA.
A 3.5% processing fee applies to credit/debit card payments. Ask us about payment questions before your appointment.

Appointment changes
Please provide 48 hours’ notice for a cancellation or appointment change. The practice’s policy lists a $50 fee for late cancellation or a missed appointment, charged to the card on file.
Lab testing and insurance
The practice offers on-site collection through HealthLab, which may bill your insurance directly. Any costs not covered by your policy are your responsibility.
Check with your insurance carrier to confirm coverage. The practice also lists a $250 cash-pay lab bundle to avoid using insurance for that bundle.
